Enrollment
To enroll in the Madison Church of Christ MMO, a completed registration form must be returned
and registration fee paid to the Madison Church of Christ MMO. In addition to the
registration form, parents must also complete and return one of the following two forms before
a child may be allowed to attend classes:
- Child's Medical Report
- Immunization Certificates or Physician's Blue Card
Registration
A registration fee of $65 will be charged for each child for fall registration. A registration
fee of $35 will be charged for each child for summer registration. This fee will be due at the time the
child is registered. The numbers registered by the time of our annual Fall Open House will be used in
planning classes and teachers for the upcoming year. Those registering after Open House will be
offered a position in the class if openings become available.
The monthly tuition required for each child will be determined based upon the number of days per
week that a child is registered to be in attendance. |
